Abstract[English]
In an aspect, a method for providing a care pathway for a subject undergoing treatment for a complex health condition is disclosed. The method comprises (a) receiving, via a user interface (UI), a query comprising one or more search parameters associated with a subject. The method also comprises (b) retrieving, from a computer server, health data of said subject from an electronic health record (EHR), wherein said health data of said subject is associated with said complex health condition, wherein said health data of said subject is retrieved responsive to said query. The method also comprises (c) outputting a care pathway for said subject, wherein said care pathway provides a time sequence of health procedures associated with at least said complex health condition, wherein said time sequence is based at least in part on said health data of said subject.[French]
Selon un aspect, l'invention concerne un procédé permettant de fournir un parcours de soins à un sujet qui suit un traitement pour un problème de santé complexe. Le procédé consiste à (a) recevoir, par le biais d'une interface utilisateur (UI), une requête comprenant un ou plusieurs paramètres de recherche associés à un sujet. Le procédé consiste également à (b) récupérer, à partir d'un serveur informatique, des données de santé dudit sujet à partir d'un dossier médical électronique (EHR), lesdites données de santé dudit sujet étant associées audit problème de santé complexe, lesdites données de santé dudit sujet étant récupérées en réponse à ladite requête. Le procédé consiste également à (c) générer une parcours de soins pour ledit sujet, ledit parcours de soins fournissant une séquence temporelle de procédures de santé associées au moins audit problème de santé complexe, ladite séquence temporelle étant basée au moins en partie sur lesdites données de santé dudit sujet.
