The invention provides systems and methods for providing real-time personalized health risk assessments and recommended interventions for a user based on a unique analysis of data collected from the user's mobile and wearable devices. More specifically, the present invention provides a cloud-based digital health platform configured to collect, harmonize, and analyze lifestyle data to generate a digital biomarker that is indicative of the user's risk in developing a health condition and predictive of the user's morbidity and mortality risk of a health condition.[French]
